Instructions:

To designate your activity, ONCB charges a CE Categorization fee ($30 if your are requesting less than 5 Contact Hours for the event). Please provide event information for us to categorize your event using the Event Categorization form below. After you have submitted the form, add the product to your cart and check out to pay the CE Categorization fee. ONCB will start to evaluate the provided information once we have confirmed payment. Designations take 2 weeks to complete.
